Initially, Fisk’s colleagues believed she had landed safely, but when they couldn’t locate her, emergency services were alerted. Investigation findings indicate that the failure of Fisk’s parachute was caused by air turbulence generated by her body. Furthermore, her non-RAF jumpsuit may have contributed to the tragic accident.
